How do I get a copy of my 1095-C form for medical insurance verification?
Form 1095‑C Availability Notice
In compliance with the Paperwork Burden Reduction Act and IRS Notice 202515, RS&H will provide a printed Form 1095‑C upon request.
You may request a printed copy using the following methods:
- Email: Benefits@rsandh.com
- Mail: RS&H, Inc., 10748 Deerwood Park Blvd, Jacksonville, FL 32256
- Phone: 904-256-2500.
Your Form 1095‑C will be furnished within 30 days of your request, or no later than January 31 of the following year, whichever is later, as required by the IRS.
